L’axe thématique DECLAN (DECLin fonctionnel et ANalyse multimodale) est un axe multidisciplinaire centré sur une thématique unique de modélisation multimodale de la fonction d’organes.

Cet axe cherche à :

  • mieux définir le phénotype du déclin de fonction d’organe
  • mieux prédire et personnaliser le risque de survenue de ce déclin
  • mieux prévenir en intervenant à l’échelle individuelle ou à l’échelle populationnelle
  • Nous avons mis en place des approches complémentaires notamment des travaux d’épidémiologies cliniques mais également des essais cliniques interventionnels. Nous développons des approches physiologiques et physiopathologiques grâce à une chambre d’hypoxie des approches de métaboliques tissulaires in-vivo.
  • Nous modélisons également le risque à partir de données issues d’évaluation et de mesures répétées dans le temps.

Les principales collaborations se font en France avec l’unité INSERM U1082, avec l’équipe d’accueil EA6314 mobilité et vieillissement de l’Université de Poitiers, et l’unité CNRS UMR 7348 DACTIM I3M.
À l’international, des collaborations ont été mises en place avec le National Institutes of Health à Phoenix Arizona (USA).

Cette étude visait à examiner l’efficacité d’un atelier d’éducation à la santé spécifique à l’incontinence urinaire en comparaison avec un atelier d’éducation à la santé générique auprès des femmes âgées vivant à domicile afin d’améliorer la continence, faire cesser la stigmatisation, et maintenir leur autonomie.

Des ateliers de 90 min ont été dirigés par une animatrice dans toute la région Poitou-Charentes. La sélection de l’atelier a été faite par tirage au sort le jour de la présentation. Chaque atelier (spécifique et générique) rassemblait une douzaine de femmes. Ce sont les résultats des deux types d’atelier qui ont été évalués dans cette recherche.

Etude ouverte, contrôlée, randomisée, en groupe parallèle évaluant l’efficacité du système de mesure continue du glucose FreeStyle Libre® sur la survenue de troubles du rythme cardiaque associés aux hypoglycémies chez des patients avec un diabète de type 2 à haut risque d’hypoglycémie et traité par insulinothérapie intensifiée . L’objectif principal est d’évaluer l’efficacité du système flash de MGC FreeStyle Libre associé à l’éducation thérapeutique spécifique sur la réduction des troubles du rythme cardiaque en comparaison à un système d’ASG capillaire associé à une éducation thérapeutique standard chez des patients atteints de DT2 à haut risque d’hypoglycémie.

Actidiane

Evaluation du bénéfice d’un programme d’activité physique de haute intensité (APHI) sur la fonction rénale des sujets diabétiques de type 2 à haut risque. Essai randomisé, ouvert, contrôlé, multicentrique ; L’objectif principal de cette étude est d’évaluer si la réalisation d’un programme structuré d’activité physique durant 2 ans encadré par un éducateur agréé pour l’activité physique adaptée permet d’améliorer la fonction rénale des patients diabétiques de type 2 à haut risque par rapport à une stratégie usuelle de simples conseils prodigués dans le cadre d’un programme d’éducation thérapeutique. Le nombre de patients qui devra être inclus dans ce programme de recherche est de 300 dont 150 dans chacun des deux groupes.
